Tight Hamstrings and Pain Bending Forward: Hamstring Complex Release

This modality teaches the release of the hamstring group, which stabilizes the pelvis and knee.

Step 1

Understanding the Problem

Hamstrings that never loosen, no matter how much they are stretched, are not short hamstrings. They are the biceps femoris, semitendinosus and semimembranosus in chronic spasm, and very often they are guarding a sciatic nerve that is being compressed higher up by the piriformis. The muscles pull continuously on their attachment at the sitting bone, the pelvis and sacrum tilt, the knee will not fully straighten, and every forward bend, toe touch or shoe tie pulls sharply at the back of the thigh. Dancers and athletes feel it as a strain that keeps coming back. Releasology releases the hamstring complex directly, and the sciatic pathway above it when it is involved, so the muscles lengthen because they are no longer in spasm.

What this release relieves

  • Tight hamstrings that stretching never changes. A muscle in spasm resists a stretch. Release the spasm and the hamstrings lengthen on their own.
  • Pain bending forward, touching toes or tying shoes. Forward bending loads the hamstrings at the sitting bone; in spasm every bend pulls.
  • Recurring hamstring strain and a pulling sensation. A hamstring that is already in spasm tears easily under load. Releasing it is what stops the strain from recurring.
  • Sharp aching at the sitting bone. The tendon at the ischial tuberosity is under constant tension from the spasmed muscle; it settles as the muscle releases.
  • A knee that will not fully straighten and stiffness walking. The hamstrings cross the knee. In spasm they hold it slightly bent.
  • Sciatica-type pain down the back of the thigh. When the hamstrings are guarding a compressed sciatic nerve, the piriformis (Modality 26) is released first and the hamstrings follow.

Frequently asked questions

I stretch every day. Why are my hamstrings still tight?

Because stretching pulls against a muscle in spasm, which resists and often tightens further. Releasology releases the spasm at the muscle; the length comes back without forcing it.

Is it my hamstrings or my sciatic nerve?

Often both. Hamstrings guard when the sciatic nerve above them is compressed by the piriformis. If the tightness came with buttock pain or pain down the leg, the sciatic pathway is released first.

I am a dancer. Will this affect my flexibility?

It restores it. Flexibility lost to spasm returns as the spasm releases, and the muscle can then be trained through its full range without the strain that keeps returning.

A client walks in complaining of pain in the back of the thigh, tightness when bending forward, or a sharp aching sensation near the ischial tuberosity, commonly known as the sitting bone.

They may report difficulty touching their toes, tying their shoes, or bending forward to pick something up. In many cases they also notice that the knee does not fully straighten, creating stiffness or discomfort when walking.

 

Stretching often provides only temporary relief because the underlying muscle spasm has not been released.

Before addressing the hamstrings directly, it is important to recognize that the piriformis must be released first if it is in spasm. When the piriformis is in spasm, it directly compresses the sciatic nerve and is the cause of true sciatica. The sciatic nerve is the nerve supply for the hamstrings, so they instantly spasm when the sciatic nerve is pinched by the piriformis. The piriformis also alters the position of the sacrum and pelvis, placing additional stress on the lumbar intervertebral discs by altering the position of the surface they directly rest on. 

 

The hamstring group consists of three muscles:

• Biceps femoris
• Semitendinosus
• Semimembranosus

 

These muscles share a common origin at the ischial tuberosity. When this tendon becomes chronically tight, the pain at the sitting bone can be quite severe. In many cases this origin release point, points near the middle, and points close to the tendon insertions must be released directly before the hamstrings will let go. They are large muscles and need more firm and sustained pressure than most muscles do in order to release.

 

The biceps femoris requires special attention because it has two heads. The long head shares the common tendon at the ischium, but the short head originates along the middle of the posterior femur. If this second origin remains tight, releasing only the ischial attachment will not fully resolve the spasm.

 

For this reason both the ischial origin and the femoral origin of the short head must be addressed to restore normal function.

 

When the hamstrings enter spasm they resist the motions they normally perform: hip extension and knee extension. These muscles flex the knee and extend the hip, so bending forward at the hip or bending the knee will both be restricted when these muscles are in spasm. 

 

Once the hamstring origins and key fibers are released, tension through the back of the thigh resolves quickly, allowing the hip to bend forward normally and the knee to extend freely again.
