Rotator Cuff and Shoulder Blade Pain Relief: External Rotator Release

The infraspinatus and teres minor are the external rotators of the shoulder, and they cover the back of the shoulder blade. When they fatigue into spasm from reaching, lifting, sleeping on the shoulder or guarding after a strain, they shorten, pull the head of the arm bone backward, and resist every internal rotation. The result is the aching behind the shoulder that most people call rotator cuff pain, pain across the shoulder blade, difficulty reaching behind the back, and a stiffness that worsens month by month until the shoulder starts to freeze. Releasology releases the external rotator complex directly, restoring the joint's position and its full range.

What this release relieves

  • Rotator cuff pain. The deep ache at the back and top of the shoulder is the infraspinatus and teres minor in spasm. It eases as they release.
  • Shoulder blade (scapular) pain. The infraspinatus covers most of the shoulder blade. Pain across the blade is that muscle, not the bone.
  • Difficulty reaching behind the back and restricted internal rotation. Reaching behind the back needs internal rotation; spasmed external rotators will not allow it.
  • Tightness at the back of the shoulder and upper back pain. The external rotators in spasm pull the shoulder blade and the arm bone backward, and the upper back guards with them.
  • Progressive shoulder stiffness and early frozen shoulder. External rotator spasm is one of the patterns that leads into a frozen shoulder; releasing it early keeps the shoulder moving.

Frequently asked questions

Is a rotator cuff tear the same as rotator cuff pain?

No. Many people have cuff tears on imaging with no pain, and most rotator cuff pain comes from the cuff muscles in spasm, not torn. When the muscle is released and the pain changes, the source was the spasm. A complete tear with weakness should be evaluated by a physician.

Why does my shoulder hurt at night?

Lying on the shoulder compresses external rotators that are already in spasm. Once they release, lying on that side stops provoking it.

Which shoulder modality do I need?

Pain behind the shoulder with restricted reaching behind the back points to the external rotators (this modality). A pinch in front on lifting points to the internal rotators (Modality 17). Loss of range in every direction points to frozen shoulder (Modality 15). Often more than one is released.
